Continuum of Care (CoC) The U.S. Department of Housing and Urban Development (HUD) allocates HUD homeless assistance grants to organizations that participate in local homeless assistance program planning networks. Each of these networks is called a Continuum of Care (CoC). HUD introduced the CoC concept to encourage and support local organizations in coordinating their efforts to address housing and homeless issues and reduce homelessness. In the 23 counties of West Tennessee, the Continuum of Care Network is the West Tennessee Housing Resource Network.

Tennessee Homeless Solutions is the lead agency for the CoC in twenty-three counties. Each year, Tennessee Homeless Solutions partners with over 40 agencies in the West Tennessee community to prevent homelessness or assist individuals and families experiencing homelessness.
